Use this form to request a location change or to request changes to an existing space allocation. Use this when there is no other associated employee technology access requests being asked; otherwise, please use the Employee Access or Terminate Access requests.
This form is used to remove ALL access for users leaving the division. Examples include transferring out of RHS, graduated with no return to RHS, and retirements. You must be an authorized requester to use this form.
