Add, change or remove employee access to accounts or service access. STOP: You must be an Approved Authorized Requester to proceed. (Please see you RHS HR Coordinator for clarification)

Employee access

New, add or change employee access to RHS systems including RHSNET, email and others. You must be an authorized requester to proceed.

Terminate Access

This form is used to remove ALL access for users leaving RHS. If you only need to remove partial access please use the Employee Access form. You must be an authorized requester to use this form.
