Employee access

New, add, change, transfer, or partial removal of employee access to SLE systems including applications, shared mailboxes, space changes, SLE meal discount plans and distribution lists. Examples include transferring neighborhoods, temporary work assignments, and a change in position. You must be an authorized requester to proceed.

Space Request

Use this form to request a location change or to request changes to existing space. Use this when there is no associated employee technology access request; otherwise, please use the Employee Access request.

Terminate Access

This form is used to remove ALL access for users leaving the division. Examples include transferring out of SLE, graduated with no return to SLE, and retirements. You must be an authorized requester to use this form.
